You will receive a reminder email or text a few days prior to your appointment with a link to begin your intake paperwork. This will help us spend more time on your treatment and less time filling out forms during your visit.
Once your evaluation is completed and your treatment plan has been discussed with you, we will set up the required number of appointments needed to get you back in good health.
